The Human Services Campus, Inc., dba Keys to Change, owns, operates, and collaborates with partner tenants to provide complementary, holistic services, all located on our 13-acre campus. Keys to Change and Key Campus partner services address the Social Determinants of Health, including:

ASSESSMENT

Keys to ChangeBrian Garcia Welcome Center: The Brian Garcia Welcome Center, operated by Keys to Change, is the lead access point to Coordinated Entry.  We provide each client with diversion (initial short and long term housing plans to avoid the cycle of homelessness), assessment, resources, and service referrals with the primary goal of resolving homelessness through a client-centered approach. Physical, Mental, & Dental Health

Brighter Way InstituteBrighter Way Dental Center: Brighter Way Institute works in collaborative and medically integrated environments to serve the needs of the most vulnerable populations, including veterans and homeless individuals. Services include everything from comprehensive exams and basic dental hygiene to dentures and implants.

Did you Know? 2,501 individuals received 10,701 dental care services (3,745 to veterans) in FY2022.

Circle the CityCircle the City: Circle the City operates Healthcare for the Homeless and provides primary care healthcare as well as medical respite in two newly renovated facilities on Key Campus.

Did you Know? In FY2022, the Health Clinic completed 10,197 medical visits, 1,590 behavioral health visits and 1,574 social services/case management visits. The Medical Respite Program completed 10,417 medical visits, 1,170 behavioral health visits, 620 physical therapy and 1,574 social services/case management visits. Circle the City also completed 2,863 COVID-19 tests and administered 778 vaccinations on the Campus.

Community Bridges, Inc.Community Bridges, Inc.: (CBI) provides the following services for our Campus Clients: Substance Abuse and Mental Health Services, Housing Coordination and Connection, Community Integration, Navigation, Case Management, Crisis Medical Services, Crisis Transition Navigators, PATH Outreach, SOAR Services. Located in the Lodestar Day Resource Center.

Did you Know? In FY2022, Phoenix Rise Outpatient enrolled 439 individuals in ongoing behavioral health and substance abuse services. And enrolled 1,849 individuals with Navigators for interim services related to the connection to ongoing services, community resources, diversion, housing and positive transitions off the Campus into community. Provided 3,422 total responses to calls for medical assistance: 2,083 EMT Campus Response Team and 1,339 CBI Navigator Response.

PATH Outreach managed the cases for 1,209 individuals; enrolled 660 individuals, completed 61 evaluations for a serious mental illness, connected 482 people to emergency shelter, and diverted 48 people out of homelessness.

ElaineELAINE: Elaine is Arizona’s first health navigation and transportation system for individuals experiencing homelessness.

Did you Know? In FY2022, Elaine provided 3,985 rides to 820 people in connection to 2,619 critical resources and services through compassionate and timely transportation. These trips to Social Determinants of Health locations include medical appointments, job and housing interviews, the grocery store and food boxes, the bank, MVD and other necessary errands.

Community Legal Services: CLS is a nonprofit law firm committed to increasing fairness in the civil justice system by advocating, litigating, and educating on behalf of Arizona’s most underserved communities. Located in the Lodestar Day Resource Center.

Community Legal Service, Inc.

Homeless Court: The Maricopa County Regional Homeless Court in Key Campus was established to resolve outstanding misdemeanor warrants for homeless individuals demonstrating a significant commitment to ending their homeless lifestyle. Individuals who owe fines for victimless crimes such as “camping” can use their time in transitional programs as community service in exchange for having their fines waived. Very often it is this burden that can keep an individual homeless. After their fines have been waived they can start fresh on their path to self-sufficiency. Located in the Lodestar Day Resource Center.

Did you Know? In FY 2022, 134 cases resolved, $152,996 fines resolved, and 27,739 community restitution hours were provided through the campus Homeless Court Advocate.

Maricopa County Regional Homeless Court

Maricopa County Adult Probation: Probation officers spend time at Key Campus in order to facilitate probationers remaining in compliance. Any individual who is on probation or who has questions about probation has easy access to the answers they need. Officers are not here to arrest people, but rather to help them move forward with their reintegration back into the community. Located in the Lodestar Day Resource Center.

Homeless I.D. ProjectHomeless ID Project: Provides the critical replacement identification services to empower individuals experiencing homelessness to rebuild their lives & end their homelessness. Did you Know? In FY2022, provided clients with 8,276 replacement identification documents needed for employment, housing, and basic benefits. Provided safe document storage for more than 4,000 clients. Documents include Arizona State IDs and Birth Certificates from all 50 states, 3 of 5 U.S. Territories and several Consular Reports of Birth Abroad. Advocated for our mission with the State Legislature, passing a bill allowing homeless and foster youth to access vital documents.

Having an actual address (rather than for “General Delivery”) is critical in helping our clients to obtain employment, Social Security benefits, medical benefits and ultimately to get into housing. Volunteer opportunities in the post office include sorting and distributing mail to clients, writing cards to clients during holidays and providing pre-stamped envelopes for clients.   • Employment Support – Transportation assistance for clients once employed, financial assistance to pay for work uniforms or shoes, help to secure certain certifications or license fees, and assistance for individuals in low-income or part-time employment in locating a higher paying job.

St. Joseph the Worker recognizes that not everyone needs all services. Clients work closely with their Job Developer or Employment Specialist to determine what level of service is right for them. Services are available in the downtown Phoenix office (located in the Lodestar Day Resource Center), at satellite locations hosted by partner agencies, and on the Mobile Success Unit.

Did you Know? In FY2022, Served 7,312 individuals with intakes and employment support services at the Campus and our satellite offices and assisted 5,399 clients in gaining and maintaining quality employment. Average pay was $15.94/hour with 68% of people eligible for benefits.

Catholic Charities Community ServicesCatholic Charities Veteran’s Outreach Center: Nearly 7% of the individuals experiencing homelessness who arrive at Key Campus are US Military Veterans. Located in the Lodestar Day Resource Center.

Did you Know? In FY2022, the Veterans Outreach Center served 231 clients with essential items, such as computer lab access to search for jobs and housing, referrals to VA services, backpacks/hygiene items/clothing.

A New LeafNew Leaf Rapid Rehousing: New Leaf Rapid Rehousing is a short term rental assistance program and when combined with Housing Case Management, individuals are more likely to maintain their housing long term. The program assists individuals with locating affordable apartments, obtaining employment, connection with community resources and benefits, understanding their rights and responsibilities as a tenant, and so much more. Located in the Lodestar Day Resource Center.

Central Arizona Shelter ServicesCentral Arizona Shelter Services (CASS): Founded in 1984, CASS is Arizona’s largest and most experienced emergency shelter provider, annually serving thousands of adults and families with shelter, a four-tiered trauma-informed case management program, and a full array of housing services (eviction prevention, rapid rehousing, and flexible financial assistance). Both our adult and family shelters operate at full capacity 24/7, 365 days a year. Each night, our adult shelter on Key Campus provides shelter and supportive services to 600 individuals, and our family shelter in North Phoenix serves up to 150 family members. Additionally, CASS has a resource center in downtown Glendale, and will soon be opening a senior emergency shelter and a shelter for those with serious mental illness in 2024.

Did you Know? In FY2022, CASS sheltered and provided housing and case management services to 4,669 unduplicated single adults on the Campus; as well as assisted an additional 2,094 individuals and families with supportive services through our other locations. CASS was able to house 196 individuals through Rapid Rehousing and divert or house 1,017 through our flexible assistance program. 

Keys to ChangeRespiro: Keys to Change opened Respiro in April 2022, adding 100 shelter beds to the Campus via a “sprung structure.” Respiro means “respite” in Spanish and is based on a wraparound concept that believes in being the stepping stone to ending homelessness. Through the process of case management, supportive services and other providers, clients are given direct attention that allows them to fill in the gaps of past attempts that did not allow them to progress towards ending their homelessness.
